An exciting opportunity has arisen for an experienced and suitably qualified Practice Nurse to lead our team of Nurses, Nursing Associate, HCAs and Phlebotomists. We are a busy and friendly training practice with 12 GP Partners, 3 salaried GP's, Registrars and ACP's, with excellent administrative support, based in rural North Devon with a patient cohort exceeding 19,300 patients.
We are looking for an enthusiastic and motivated prescribing nurse who has the ability to lead, develop and support our nursing team and deliver patient care to high clinical and operational standards. You will ideally have qualifications to specialise in Diabetes and have knowledge and experience of dealing with other chronic diseases in addition to other Practice Nurse tasks.
This role is also responsible for leading the annual flu campaign, including the planning and coordination of clinics, coordination of staff, to achieve high vaccination levels in our eligible patient population.
It is essential that you have recent knowledge of working in Primary care and experience of leading and managing staff will be key to a successful application.
The post is for 37.5 hours per week. Salary will be according to experience.

Main duties of the job
Responsible for the operational day to day running, management and development of the practice nursing team, consisting of nurses, HCAs and Phlebotomists under the guidance of the GP Treatment Room Lead.
To work as part of Brannam multidisciplinary team to develop an effective, quality driven Treatment Room service, ensuring reference to practice agreed protocols and guidelines to deliver nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The lead practice nurse will be responsible for a number of clinical areas such as health promotion and chronic disease management, specifically taking a lead in Diabetes.
Working in accordance with all essential protocols and registration requirements, the post-holder will provide assessment, treatment, screening, health education services and advice to patients including all tasks normally undertaken by an experienced nurse.
The post-holder will have the required level of training and competence and will work within their prescribing qualifications.

Job responsibilities
Management of Chronic Disease: Provide a nurse led specialist management service for patients with diabetes, and in one or more of the following areas: Sexual Health, Rheumatology, Hypertension.
Diagnose, monitor and develop individual management plans, agreeing these as appropriate with the patient and other health professionals.
Patient health checks: Working with patients to develop a management plan where health problems or potential health problems are identified. Taking responsibility for and the practice lead for health checks for patients with learning disabilities.
Wound Management and Care: Assessing routine wounds including trauma. Prescribing to support good wound management in complex situations.
Emergency Management: Competent in anaphylaxis and resuscitation techniques.
Family planning and sexual health: Provide comprehensive service and advice to patients including available methods of contraception, support patients attending for emergency contraceptive. Refer patients as appropriate. Perform interval health checks, provide advice reducing risk of sexually transmitted diseases, undertaken opportunistic screening.
Women's Health: Advise on national and local cervical cytology and mammogram screening programmes. Undertake cervical cytology sampling, promote breast self-awareness, demonstrating self-examination and provide supporting literature.
Men's Health: Raise awareness and give basic advice on issues covering: testicular self-examination, incontinence and prostatic disease, family planning, sexual health, make appropriate referrals.
Risk Assessment: Recognising issues and gathering sufficient information to refer e.g. Drugs; domestic violence; child protection; vulnerable adults; social problems.
Health Promotion: Deliver opportunistic health promotion where appropriate, liaising and providing advice to other members of the practice team in terms of campaigns and promotion.
Vaccinations and Immunisation (adult & child): Administer injections under an individualised prescription or PGD. Act as practice lead for the Annual Flu programme including the organisation of clinics, promotion and invitations to the eligible patient population, liaising with other members of the practice team as appropriate.
Infection Control: Support patients and colleagues in adopting sound infection control measures. Apply infection control measures within the practice according to local and national guidelines, to include: universal hygiene precautions, the collection and handling of laboratory specimens, segregation and disposal of waste materials, decontamination of instruments and clinical equipment, reporting and treatment of sharps injuries, dealing with blood and bodily fluid spillages, educate colleagues in adopting sound infection control measures.
Managerial: Provide leadership and management of the nursing team including:
Regular assessment of the performance of the Nursing Team. Undertake annual appraisals of all members of the nursing team.
Training: Encourage and organise attendance to relevant recognised courses, ensure all competencies are up to date, and that all members are compliant with mandatory and relevant clinical training. Manage induction and training, provide ongoing support to new members.
Organise the Nursing Team rota, ensuring sufficient cover at all times. Provide clinical supervision of the Nursing team and act as clinical expert.
Plan, chair and minute regular nurse meetings.
Support nursing team through revalidation.
Deal with any problems reported by the Nursing team.
Lead and monitor on the development and implementation of policies, standards and guidelines across nursing services.
