The post holder will be supported by theLead Practice Pharmacist. They will also engage with pharmacy colleagues in YeadonPCN and across Leeds for peer support.
About us
Aire Valley Surgery is a GP Practice based over two sites in Rawdon and Yeadon in North Leeds. A training practice with 15000 patients, we accommodate 18 GPs including trainees and we have a growing Nursing Team, Physician Associates, Advanced Nurse Practitioners as well as supporting Medical Students and Trainee Advanced Clinical Practitioners.
There are 3 practices in our Primary Care Network, however the Pharmacy Team is dedicated to work in the one practice and does not cover other surgeries. We have a Lead Pharmacist, a Band 8 Pharmacist, a Band 7 Pharmacist, a Pharmacy Technician and 2 Prescriptions Clerks. This role is to cover some of the tasks from a maternity leave vacancy of one of the team who will be away for 9-12 months.

The post holder will be supported to develop their role tobecome a non-medical prescriber, if that qualification is not already held,and will be required to enrol on the General Practice Pharmacist TrainingPathway from CPPE, unlessan equivalent qualification is already held or exemption is agreed by CPPE.
Key duties and responsibilities
Please note this is a list of options, it is not exhaustive, whichoptions are deployed will be decided by the Lead Practice Pharmacist and thePractice Manager. This role is patient-facing in nature.
1.Patient facing Clinical MedicationReview
Undertake remote clinical medication reviews with patients and workwithin your scope of practice as a non-medical prescriber to implement anynecessary changes (or produce recommendations for/refer to other prescribingpharmacists, nurses and/or GPs to implement if outside your scope). Thesereviews could be cohort based, in care homes, polypharmacy or any other arearequired by the practice, within the pharmacists competence.
Undertake clinical audits of prescribing in areas identified byyourself and agreed by the practice or as directed by the practice, feedbackthe results and implement changes in conjunction with the relevant practiceteam.
Identify cohorts of patients at high risk of harm from medicinesthrough computer searches. This might include risks that are patient related,medicine related, or both. Put in place changes to reduce the prescribing ofthese medicines to high-risk patient groups
Implement changes to medicines that result from MHRA alerts, productwithdrawal and other local and national guidance.

Provide a telephone help line for patients with questions, queries andconcerns about their

Answer relevant medicine-related enquiries from GPs, other networkstaff, other healthcare teams (e.g. community pharmacy) and patients withqueries about medicines. Suggest and recommend solutions. Providing follow upfor patients to monitor the effect of any changes.
7.Signposting
Ensure that patients are referred to the appropriate healthcareprofessional for the appropriate level of care within an appropriate periodof time e.g. pathology results, common/minor ailments, acute conditions, longterm condition reviews etc.
8.Repeat prescribing
You will be asked to contribute to the repeat prescribingreauthorisation process by reviewing patient requests for repeatprescriptions and reviewing medicines reaching review dates. Ensure patientshave appropriate monitoring in place when required.
9.Information management- Dependent onexperience
Analyse, interpret and present medicines data to highlight issues andrisks to support decision making.
10.Care Quality Commission
Work with the general practice teams to ensure the practices arecompliant with CQC standards where medicines are involved.
11.Public health
Support public health campaigns. Provide specialist knowledge on allpublic health
programmes available to the general public.
12.Collaborative working arrangements
Work collaboratively with the Practice Manager, Partners, PrescribingLead, Lead Pharmacist, Lead Nurse and other members of the pharmacy team.
Participate in the practice MDT.
Liaise with the Integrated Care Board (ICB) in Leeds MedicinesOptimisation team to benefit from peer support, and on prescribing relatedmatters to ensure consistency of patient care and benefit
Engage with the Leeds Practice Pharmacist and Technician Network forpeer support.
