An exciting role for an Advanced Clinical Practitioner has been created to join the dynamic CTT team that covers Barking & Dagenham, Havering, & Redbridge. Community Treatment Team (CTT) aim to avoid unnecessary admissions to the acute hospital settings by managing patients in crisis in their own homes. The post holder will deliver high quality holistic nursing as part of a multidisciplinary team within the accident and emergency and community setting, working with patients towards rehabilitation, self-management and independence throughout the continuum of care.
Job Purpose:
The post holder will use advance assessment and diagnostic skills to assess and treat patients. The post holder will be an autonomous practitioner, collaborating with colleagues in primary care, community and hospital settings as appropriate.
Key Responsibilities:
1. Be a Qualified Advanced Nurse Practitioner
2. Act as a role model
3. Be responsible for undertaking comprehensive health assessments, physical examination and ordering tests for patients in Rapid Response
4. Use advanced clinical decision making and assessment skills, coordinate care planning and initiate referrals to diagnostics for appropriate tests for this population.
5. Be involved in establishing delivering and leading a comprehensive service within community services
6. Play a leadership role within the team and work with external stakeholders such as London Ambulance Service and Social Services to develop new ways of working and through networking increase the number of referrals from the acute and community.
7. Lead on service development
Clinical Practice:
1. Be professionally and legally accountable for all work undertaken and to practices at an advanced level of professional autonomy and accountability that is within national guidelines and Nursing and Midwifery Council’s Code of Conduct.
2. Provide clinical leadership and maintain credibility through practice and professional update.
3. Practise evidence based nursing care to ensure high quality care.
4. Act as an expert nurse and resource for patients, families, and staff.
5. Admit, discharge or transfer patients to and from the service as their needs require.
6. Undertake advance clinical assessments of patients referred to the service to enable prompt assessment, diagnosis and treatment according to agreed policies, protocols and guidelines.
7. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
8. Order and interpret blood tests and initiate appropriate action as required.
9. Be competent in using a range of diagnostic tools including bladder scanner, point of care testing & ECG
10. Maintain and develop own repertoire of advanced clinical assessment skills including physical examination, assessment of continence, mental health screening, nutritional status, falls assessment.
11. Prescribe medication within own level of knowledge and competence and within the scope of the Trust’s Policy for Non-Medical Prescribing qualification.
12. Support staff as required in the management of challenging situations.
13. Complete documentation relating to patient care and ensure that staff maintain and update documentation as changes occur according to Trust policy.
14. Follow the process through the planning and evaluation stages of patient care and to effectively document this process in the patient records.
15. Liaise and work closely with relevant multi-disciplinary team members, including General Practitioners and therapists in the provision of professional service.
16. Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
17. Take responsibility for own learning and performance including participating in clinical supervision and acting as a role model, as well as continuous preparation for annual personal appraisal and development review.
