Due to our dedicated practice nurse retiring in March, Lincoln House Surgery is looking to welcome an experienced, qualified general practice nurse to assess, plan, develop, implement and evaluate treatment programmes for our patients. This role is offered on a full or part-time basis and for the right candidate, we are looking for a minimum of 3 days per week, 7.5hrs per day, Monday to Friday between the hours of 9am - 6pm on a rota basis. This can be discussed at interview. We are a dual branch practice and you would be expected to work at either branch when required due to the needs of the business, Highfield and Lincoln House Surgery. The main responsibilities of the role include: Undertaking health assessments, administering medication and treatments and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met. Health and well-being promotion plus individual person centred plans for patients with/at risk of a long term condition. Managing and treating long-term conditions in line with national and local policies and practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments. Supporting and managing health needs of women presenting for family planning and cervical cytology consultations. Implementing and participating in vaccination and immunisation programmes for both adults and children including patients travelling abroad. Meeting the needs of patients presenting for opportunistic wound care. Hypertension review and management. Running Asthma, COPD, Diabetes clinics and reviews. New patient and health checks. About the Candidate If you are a practice nurse with GP practice nurse experience, looking for an exciting new role then we would love to hear from you. You will have the following skills and qualifications: A relevant nursing degree and at least 1 years post qualification experience in a general practice setting. Current NMC registration with an active PIN. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications. Competence to undertake Asthma, COPD and Diabetic clinics / reviews and hold relevant certificates. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations (Adult & Child), assisting with minor surgery, dressing and wound care and recording spirometry. Compassionate, empathetic approach and demonstrable kindness in all aspects of your work as well as valuing the dignity of our patients, relatives, carers and colleagues. Knowledge and application of supporting long-term conditions. Insight and understanding of current issues in nursing and NHS.
