The Advanced Nurse Practitioner (ANP) at The Village Surgeries Croston & Eccleston plays a vital role in delivering high-quality, patient-centred care. As a highly skilled and experienced healthcare professional, the ANP has demonstrated advanced clinical competency and expertise in diagnosing, treating, and managing a broad range of health conditions. The ANP will be able to assess patients, make independent clinical decisions, and initiate appropriate interventions, including prescribing medications and ordering diagnostic tests. ANPs at The Village Surgeries Croston & Eccleston work closely with patients, developing long-term relationships built on trust and collaboration. By offering expert care without the need for GP referral, the ANP ensures that patients receive timely treatment, reducing delays and enhancing overall healthcare access. Additionally, the ANP evaluates treatment outcomes and refers patients to specialists or other healthcare services when necessary, ensuring comprehensive care. This role offers a unique opportunity to provide holistic, patient-focused care, empowering patients to achieve and maintain optimal health within the primary care setting. You may be required to do home visits.
Main duties of the job
In this role you will work independently, making professional, autonomous decisions. The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care. Involve independent prescribing with the scope of professional practice. You will be responsible for making necessary referrals, requests for investigations.

The Village surgeries Croston & Eccleston is based over 2 sites. We provide Medical Services for our population which is just over 10,000 patients. Our team includes 2 GP Partners, 3 salaried GP's, first contact physio, Paramedic, Pharmacist, 4 ANP's, 3 Practice Nurses, a nurse associate, 2 HCA's, 2 secretaries, and a team of Care Navigators.
Job responsibilities
The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
* Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
* Takes an accurate history, building rapport towards a therapeutic relationship.
* Implement and evaluate individual specialised treatment plans for chronic disease patients.
* Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health.
* Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
* Process and interpret pathology and other test results as required.
* Maintain accurate clinical records.
* Ensure SNOMED CT codes are used effectively.
* Maintain chronic disease registers.
* Prioritise health issues and intervene appropriately.
* Recognise, assess and refer patients presenting with mental health needs.
* Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required.
* Contribute to practice targets (QOF, etc.), complying with local and regional guidance.
* Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.).
* Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual).
* Support the clinical team with all safeguarding matters in accordance with local and national policies.
* Provide supervision and support to other members of staff such as Healthcare Professionals, Practice Nurses and Nurse Associates.
* Attend clinical, MDT and practice meetings.
* There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.
* Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
* Home visits - if and when required.
Person Specification
Qualifications
* Registered General Nurse - currently registered with Nursing and Midwifery Council.
* Recognised ANP qualification.
* Qualified Independent Nurse Prescriber on the NMC Register.
Experience
* Experience of working in a GP Practice.
* Experience of working on Emis.
