Main duties of the Community/District Nurse:
* You will be working as part of the Primary Care at Home team predominantly on Unplanned Care to triage and assess referrals into the service.
* Be accountable for initial history taking, clinical assessment, triage and onward referral.
* Expected to be flexible to cross cover.
* Through face to face patient visits, you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.
* Working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.
Requirements of the Community/District Nurse:
* RGN qualified/NMC registered with valid live pin.
* Recent UK nursing experience.
* Must have Community/District nursing experience.
