We have a full time position available withinthe Trowbridge Community Team, as we are looking for inspirational RegisteredNurses to deliver a high standard of quality care to patients within thecommunity setting. Wiltshire Health and Care is proactive in developingcommunity specific training and can offer a supportive and friendly environmentin which to learn new skills.
Community Nurses deliver care in patients homes andresidential homes as part of a Multi-Disciplinary Team, linking closely withthe GP Practices and other community services in the local areas. We work withthe aim of preventing unnecessary admissions to hospital and in assistingpeople to manage their conditions in their own homes. The Trowbridge CommunityTeam are a multidisciplinary team including Nurses, Therapists, Clinical Leads,the Home First Team, Care Coordinators and other staff.
Main duties of the job
The team is based in Trowbridge Community Hospital andoperates a shift pattern from Nursing staff will be expected towork weekends and will be expected to cover early and late shifts., Evidence ofup to date clinical professional development is essential and experience infront line NHS community patient care is preferable.
Applicants need to have excellent clinical skills, welldeveloped communication skills and be able to work under pressure. The role isdemanding but rewarding, and we are looking for an enthusiastic, motivated andhardworking individual who can work flexibly and efficiently. We are a busy, friendlyteam, who would welcome the opportunity to work with a new member of staffwilling to contribute to the on-going development of the services we provide.
