Job Description
We are seeking a dedicated and compassionate Social Care Assessor to join an established Adult Social Care team at Liverpool City Council. This part-time role involves working with adults (18+) within the Fiveways Locality Team, based in Wavertree, L15. 1. Hours : Part-Time, 17.5 hours (Wednesday PM to Friday)
2. This role is paying at £19.40 LTD inclusive of holiday pay
3. Initial 3 month contract with the possibility of extension after that
Responsibilities
4. Conduct strength-based assessments with individuals to identify their needs.
5. Formulate tailored support plans, ensuring services are person-centered and promote independence.
6. Liaise with professionals from various sectors, ensuring a multi-agency approach to care.
7. Make referrals to appropriate services based on the individual's specific requirements.
8. Ensure all assessments and plans are documented accurately and completed within expected time-frames.
Requirements
9. Experience in social care, particularly in adult services, is essential.
10. Strong knowledge of strength-based assessments and person-centred support planning.
11. Excellent communication skills, both verbal and written, with the ability to engage effectively with individuals, families, and professionals.
12. Ability to work independently, manage your time effectively, and prioritise tasks.
13. A compassionate and empathetic approach to supporting vulnerable adults.
