Job summary We are seeking to recruit an experienced and efficient Co-ordinator to join an exciting new development of an enhanced crisis / intensive home treatment 7 day a week service. This is a brand new integral role co-ordinating the front door, referrals, patient information and data and working to the service performance timeframes. You will be part of an exciting development to give children and young people living in Sussex a more appropriate integrated range of services to meet their clinical needs. As the Patient Flow Co-ordinator you will work in partnership with the clinical team and Provider Collaborative to assist young people accessing acute care at the right time and in the right place. This will involve overseeing young people in an inpatient bed and also being seen in the community including intensive home treatment. The aim of the Sussex CAMHS 'hospital at home' is to provide an alternative to admission to support the local network around the young person (aged 12-18) whilst they are in the acute stage of mental health difficulties. You will be responsible for managing data, patient information and overseeing the patient journey through acute care via data management and other essential administrative processes.
