As a member of the Pharmacy Medicines Optimisation team, you will be responsible for providing a comprehensive, ward based, pharmacy service by:
* Talking to patients to assess that any of their own medication they may have brought with them is safe and suitable for use and record this on the electronic medicines prescribing system.
* Make sure that the patient has a supply of medication available on the ward to use whilst admitted for treatment, to avoid missed doses, by ordering medicines using the electronic prescribing system.
* Liaising with other members of pharmacy and ward staff, at all levels, to contribute to the effective running of the service.
* Dispense prescriptions on the ward for patients to take home, to support a timely discharge and assist the Pharmacy team in counselling patients on new medications provided.
* Transfer medication between wards as required.
* Support nursing staff with medication orders and queries.
* Support the training and development of Pharmacy Support Workers, Trainee Technicians, Pharmacy Technicians, Trainee and Foundation Pharmacists.
The Pharmacy Department operates a 7 day service throughout the year and as part of the role you will be expected to participate in the weekend and Bank Holiday rotas. The role may also involve some cross-site working and late nights.
The successful candidate must hold an NVQ Level 2 certificate in Pharmacy Services or a GPHC recognised equivalent qualification and have experience of working in a pharmacy environment, either in community or hospital. You will need to be enthusiastic, self-motivated and passionate about patient care. You will have exceptional attention to detail and be able to prioritise the daily workload, along with excellent organisational and communication skills.
