The role aims to assist individuals affected by the opioid epidemic in securing and maintaining occupations that will provide both a living wage and a solid direction forward.
Responsibilities
1. Build a professional one-on-one working relationship with participants to aid in eliminating challenges that hinder obtaining and retaining employment, including assessment, individualized attention, and coordination.
2. Provide workshops to participants residing in rehabilitation centers.
3. Recruit a minimum of 150 participants to attend workshops.
4. Work directly in approved rehabilitation centers to provide services related to removing barriers to employment.
5. Refer customers directly to the PA CareerLink for post-rehabilitation services as part of their treatment plan.
6. Responsible for participation tracking and monitoring data entry into CWDS in accordance with activity codes and barrier identification and resolution.
7. Update the CareerLink Referral Spreadsheet Report to provide status updates on new participants.
8. Assist in the preparation of weekly project reports (participation, placement, etc.).
9. Create and maintain an environment that promotes opportunities for customers to hope for the future, accept themselves, and provides tools for facing adversity.
10. Provide referred customers with information on recovery-friendly employers in the region.
11. Ensure recording keeping of all customer outreach and contacts through internal tracking systems.
12. Oversee participant activities and assist with workshop schedules.
13. Provide consistent and comprehensive case management and follow-up.
14. Provide individualized assessment of work, family, and personal development needs and opportunities.
15. Provide referrals to appropriate community service or social service agencies when needed.
16. Maintain an organized database of clients and keep detailed documentation of all contacts with clients.
17. Coordinate participant post-exit retention services and accommodate participant work schedules.
18. Document all participant contacts and activities in case notes and ensure that post-placement contacts are made as required by specific contract.
19. Share information with appropriate staff and/or funding agencies.
20. Maintain participant files as specified by the contractor.
21. Ensure participants follow through with planned activities as outlined in the contract with the funding agency.
22. Ensure that positive outcomes and goals are met as required by the contract.
23. Other opportunities as available.
Qualifications
1. Ability to prioritize tasks and document visits and contacts.
2. Demonstrated telephone, computer, and written communication skills.
3. Ability to maintain flexibility as needs of the contract or contractor require.
4. College degree or equivalent experience required.
5. A working knowledge of Microsoft Office.
6. Valid Driver’s License, vehicle, and clean driving record required.
