Job Title: Hospital Discharge and Assessment Team Locum Social Worker
Location: Wiltshire Council
Rate: £35 per hour
An exciting role has become available in Wiltshire. They are looking for a social worker to join their Hospital Discharge and Assessment Team.
What will your responsibilities be?
You will undertake assessments, determine care packages/support plans, and manage a large and complex caseload to maintain or improve the wellbeing of clients. You will also professionally supervise and support other colleagues. You will need hospital social work experience and also skills to manage safeguarding enquiries and complex case work.

Qualifications and Experience:
* To be a Social Worker registered with Social Work England with a completed ASYE year or equivalent demonstrable experience in a statutory social work setting
* Have experience working within Adult Social Care, including working in a multi-agency and court environment
* Have knowledge and understanding of the legislation, models, and theories relevant to Adult Social Work
