An exciting opportunity has arisen for a band 5 cardiac rehabilitation nurse to join the trust wide community cardiac rehabilitation team.
The post holders base is to be negotiated, however they will be expected to travel across Northumberland and North Tyneside to deliver care.
A car driver is therefore essential.
The post holder will deliver cardiac rehabilitation to patients in their homes, clinics and leisure venues. This involves lifestyle education as well as practical exercise sessions. A good understanding of cardiology, procedures and treatments and the effect on individuals quality of life is required to ensure delivery of evidence based high quality care. The successful applicant must be able to demonstrate the ability to take on new challenges and actively participate in developments within the service.
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. To work as a community cardiac rehabilitation nurse, providing a resource within Northumbria Healthcare NHS Foundation Trust maintaining and improving communication pathways between primary, secondary and tertiary care.
To ensure a coordinated and seamless approach for cardiac patients and their families, linking in with partnership organisations.
To manage patients with a wide range of cardiac conditions, assessing, planning, implementing and evaluating care.
