CareNavigator
Responsible to Operations Manager
Our Care Navigators provide the first point of contact for our patientsand act as a focal point of communication between patients, doctors and othermedical staff.
Care Navigators are key members of The Quarter Jack Surgery team.They are trained to work with all the resources available to them which mayinclude signposting patients to another service, offering self help advice orbooking with an appropriate clinician in the practices multi-disciplinaryteam. This work is often referred to as care navigation.
The following are the core responsibilities of a CareNavigator. There may be on occasion arequirement to carry out other tasks; this will be dependent on factors such asworkload and staffing levels.
