An exciting opportunity has arisen for a Mental Health Nurse Band 6 to work within the HPFT Integrated Care Team within the Hertfordshire area. Our Integrated Care Mental Health team (ICT) specializes in working with people who have long-term physical health conditions, including frailty, which may impact their mental health. Our team aims to support service users to continue to live independently in their own home.
This is a unique position for someone who enjoys teamwork, challenges, and variety in their day-to-day work. You will work collaboratively with other health colleagues with service users who have complex frailty needs impacting their mental wellbeing. This position is based in the Hertfordshire area, supporting our service users with mental health and wellbeing in their own homes, including nursing and residential homes.
Main duties of the job
You will be expected to:
* Visit service users identified under the frailty pathway in their residence to complete a holistic assessment of their physical and mental health needs.
* Take responsibility for the assessment of care needs, development, implementation, and evaluation of programmes of care, and management of a defined caseload, including liaison with other agencies and supervision of staff and/or students.
* Work in partnership with other healthcare professionals and trusts, aiming to make healthcare more preventative and supporting service users with multiple long-term or complex conditions to remain at home.
* Hold a caseload of service users, ensuring the care plan is met at all times.
* Be part of a supportive multidisciplinary team of mental health nurses, mental health OTs, and the Psychology team.

The post holder will:
* Be responsible for the full assessment of care needs and risk assessment of service users presenting with a variety of clinical conditions.
* Develop, implement, and evaluate programs of care without direct supervision.
* Manage oneself in a professional manner, responding to requests and inquiries in a timely fashion.
* Respond to any crisis that a service user may advise of, promoting independent living in the community.
* Administer and/or supervise medications as required, having sound knowledge of medications and their effects.
* Provide education, advice, and guidance to service users and their families.
* Participate in providing quality care in line with local and national guidelines.
* Act as a role model in providing services for people with mental health problems.
* Participate in audit/research projects associated with the service.
* Challenge practices that compromise high-quality care for older people.
* Advise the Team Manager of any resource shortfalls.
* Partake in continuing professional development that benefits the development and quality of the service.
* Remain updated with local and national guidelines and policies.
* Partake in home visits with other professionals.
* Partake in clinical and management supervision.
Person Specification
KNOWLEDGE, TRAINING AND EXPERIENCE
* Degree, diploma or equivalent in registered Nursing (Mental Health Nursing).
* Registration with Nursing and Midwifery Council.
* Two years post-registration experience, with at least one year in Older Adult Mental and/or Frailty health care.
* Pre or post-registration training/short courses in mental health.
* Community and inpatient nursing experience.
* Physical healthcare training.
* Experience of working with clients with Delirium.
* Experience of working in primary care.
AREAS OF EXPERIENCE AND KNOWLEDGE
* Knowledge of legislation related to mental health (e.g., Mental Health Act, Mental Capacity Act, Care Act, Care Programme Approach).
* Good knowledge of Microsoft Word, PowerPoint, and Excel.
* Ability to manage distressing and challenging situations.
COMMUNICATION SKILLS
* Excellent communication skills, verbally and in writing.
* Ability to feedback and discuss cases within the multidisciplinary team.
* Excellent documentation of duties performed.
* Confidence to work autonomously in the community.
ANALYTICAL SKILLS
* Good range of problem-solving skills.
* Good range of information gathering skills.
* Ability to assess and manage risks.
* Experience in working with service users with personality disorder.
* Maintaining a positive non-judgmental attitude and therapeutic optimism towards working with people with personality disorder.
