Specialist Pharmacist, Diabetes and Endocrinology

This is an exciting opportunity for you to join our dynamic, innovative and friendly team dedicated to improving our ward based Clinical Pharmacy services. In this key post you will be working as part of an established multidisciplinary team and be responsible for providing a high quality Clinical Pharmacy service to the endocrinology ward.
The successful candidate will be expected to work Monday - Friday and participate in weekend (Saturday and Sundays) and Bank Holiday rotas.
Main duties of the job
As an enthusiastic and patient oriented Pharmacist, you will combine expert opinion and expert practice at the bedside to benefit patient care, and work at the forefront with the endocrinology team to help us implement, deliver and continually improve medicines management to the endocrinology ward. You will be responsible for reconciling medicines prescribed, promote medication reviews and support the discharge process; thereby facilitating high quality patient outcomes whilst reducing prescribing errors and ensuring patient safety. You will apply your expert endocrinology and diabetes knowledge to patient care and form a part of the MDT. The role will be overseen by the Lead Specialist Pharmacist, Diabetes & Endocrinology.
You will need post-registration experience relevant to hospital Pharmacy and will have completed or be working towards completing the Post Graduate Diploma in General Pharmacy Practice or equivalent qualification. You will need excellent communication, interpersonal and time management skills and will need to enjoy working as part of a multidisciplinary team.

Qualifications
* Pharmacy Degree
* GPHC registered
* Certificate in pharmacy practice
* Post Graduate Clinical Diploma
* Independent prescriber
* Previous experience in diabetes and endocrinology
Experience
* Hospital Pharmacy Experience
* Good, broad clinical knowledge and ability to apply to a ward and dispensary setting
* Some experience of providing training, mentorship and supervising staff
* Documented evidence of completed project work, audit and practice research
Skills
* Ability to work quickly, calmly and accurately under pressure

Salary: £51,883 to £58,544 a year including HCAS
