Job Description
Based, within the Learning Disability Service, at home and Claverhouse East, Jack Martin Way, you will work full time, 37 hours per week. You will work with adults aged 16+ with a learning disability and/or autism. This post may involve work in relation to transition planning for young people and for individuals who are subject to Adults with Incapacity Orders and Adult Support and Protection procedures.

Requirements
You will have a Degree in Social Work, CQSW, Dip SW, CSS (with appropriate option).
Responsibilities
You will assist in maintaining vulnerable people in the community by providing a range of care management tasks. These include:
1. The assessment of individual need
2. The negotiation of flexible packages of care
3. The monitoring and reviewing of individual care packages
The Individual
You will have experience and knowledge of social work values, a working knowledge of assessing needs, working with other professionals and third sector agencies, and an interest in working with Learning Disability. Knowledge of community care is essential. You will have good verbal and written communication skills and the ability to work effectively with a variety of other agencies. IT/PC competence, knowledge of Person Centred Planning, Personal Outcomes Approaches, and the ability to prioritise workloads and meet challenges and deadlines are also essential.
You will be able to build good, trusting relationships with service users, colleagues, and other professional staff and be flexible in your approach to work. You will be able to work effectively as part of a team. Knowledge of Children and Adult Support and Protection legislation and Adults with Incapacity Legislation is also required. You must be able to register with the Scottish Social Services Council within the specified timescale.
This post is considered Regulated Work with Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check, prior to a formal offer of employment being made by Dundee City Council.
