PURPOSE OF THE ROLE:
The Transformation Manager of Neighbourhood Development will work closely with the Associate Director of Neighbourhood Development, directors, and senior managers for their area (internally and externally) to ensure oversight and delivery of joint responsibilities. They will lead in the areas of:
1. Supporting the development and the delivery of the strategy for Neighbourhoods including Community Services, Personalised Care, Long Term Conditions, End of Life Care, VCFSE, and Ageing Well, that in turn delivers the outcomes required for the people of Somerset when accessing care.
2. Support and lead on the delivery of the transformation agenda for pathways of care within Neighbourhoods.
3. The postholder will support the Associate Director in developing and delivering the commissioning, service re-design, and transformation in all areas of Neighbourhoods, working collaboratively with system partners.
The Transformation Manager will be responsible for ensuring that transformation delivery plans are developed, maintained, and delivered, in line with organisational governance arrangements, to support effective service redesign, commissioning, and performance management in partnership with system colleagues. The Transformation Manager will also support the Associate Directors within the Strategic Commissioning team in work relating to their assigned locality, specifically around place-based commissioning at a neighbourhood level. The Transformation Manager will support collaborative working across organisational boundaries by building strong and trusting relationships with key system partners to improve outcomes for the population of Somerset.
KEY RESPONSIBILITIES OF THE ROLE:
Working with all directorates within the ICB and with partners across the system, ensuring a good understanding of local need and best practice in respect of all commissioned services in line with agreed priorities for Somerset.
To lead on pathway development, ensuring a whole systems approach is undertaken and supporting the delivery of programmes of work identified.

Working with contracting colleagues to ensure key deliverables are monitored and necessary actions taken where issues are identified. To interpret and implement relevant policies, strategies, and guidelines. To assist in the development of operational plans for the ICB. To work with partners to investigate and respond to patient queries relating to ICB commissioned services, as well as supporting the implementation of service improvements where systemic issues are identified.
To provide and receive information and reports including presenting information to internal and external stakeholders. To support the identification of key risks facing commissioned services and ensure that appropriate risk management, mitigation, and reporting arrangements are put in place.
Provide support to the Associate Director within their designated locality to enable localities to focus on the development of models of care, working in partnership with the quality and primary care teams within the ICB to ensure models and pathways adopt best practice. As directed by the Associate Director, develop and implement joint commissioning and co-commissioning in relevant area of responsibility or locality area to deliver the greatest benefit to the people of Somerset.
Support the production of commissioning plans in line with national and local timescales and are monitored regularly and appropriately. Engage with wider stakeholders and the Somerset population on how programmes of work are co-designed and co-produced, working closely with the Communications and Engagement team.
Provide expertise in designated area of commissioning responsibility and support the development of the team, encouraging the adoption of an inclusive and supportive working environment. Work with a range of stakeholders including patients and providers, clinicians, and colleagues both within the organisation and externally, and across appropriate levels of seniority.
Develop, lead and maintain highly collaborative relationships with a wide range of key stakeholders, including the regulator, to secure full engagement at all stages of the programme of work. Work with staff and clinicians across the system to ensure that policy is embedded throughout the organisation and that the impact on other functions is identified and addressed in a developmental and integrated way.
Use effective communication, negotiation, and influencing skills to enable effective change management with stakeholders at all levels who may hold differing and contentious views. Identify areas of opportunity for efficiency and productivity pathway improvements that contribute to the overall financial plan within the system.
Develop, monitor, implement and evaluate programme project plans and produce comprehensive reports. Matrix work with other Locality and Transformation teams and relevant teams/individuals across the ICB and the system.
The role is responsible for dealing with, or advising on, highly confidential information and explaining highly complex, highly sensitive, and potentially unwelcome information to a wide range of audiences while overcoming significant barriers to resistance. The portfolio will be regularly reviewed by the Associate Director and Director of Localities and Strategic Commissioning and the focus, priorities, and portfolio will be adapted to meet organisational and service needs.
