What you will do: - Undertake strengths-based assessments of adults in the hospital setting under the Care Act 2014. - Collaborate with NHS and community colleagues to coordinate effective discharge planning. - Support service users and families to develop and implement personalised care and support plans. - Facilitate prompt and safe hospital discharges, minimising delays and promoting independence. - Assess risk and capacity, implementing safeguarding procedures as necessary. - Provide expert advice and guidance on social care issues to hospital staff and families. - Maintain comprehensive case notes, ensuring compliance with statutory requirements and professional standards. - Participate in multi-disciplinary team meetings, discharge planning meetings, and case conferences. - Apply your knowledge of relevant legislation, including the Mental Capacity Act 2005, Deprivation of Liberty Safeguards (DoLS), and Continuing Healthcare (CHC) framework. Requirements: - Qualified Social Worker with current Social Work England registration. - Experience working with adults in hospital discharge, community care, or similar settings. - Solid understanding of relevant legislation and statutory responsibilities. - Strong assessment, planning, and communication skills. - Ability to work effectively within a multidisciplinary team. - Proactive, person-centred, and outcome-focused approach. - Enhanced DBS check (or willingness to undergo one).
