Job overview
An opportunity has arisen for Mental Health Practitioner posts within the Havering Older Adult Mental Health Team.
We are looking for an experienced Mental Health Nurse, Occupational Therapist or Social Worker who can rise to the challenge of working within the busy Older Adult MHT and support the continued development of the service.
The successful candidate will use their evidence based clinical skills to manage a caseload of service users with frailty and functional mental illness or a primary dementia.
Main duties of the job
The applicant will need to have a minimum of one year post-registration clinical experience in mental health where a degree of autonomous working has been required. They should be able to demonstrate the ability to work on their own initiative, have good time management skills and be able to prioritise workload effectively.
You will care coordinate a caseload of service users that have complex and challenging needs. This will involve assessment of care needs, planning and reviewing them, enabling service users to maximise opportunities for recovery.
You will work closely within a multi-disciplinary team and other agencies to deliver high quality care and recovery focused health and social care interventions that includes managing risk and the safeguarding of adults at risk.
All band 6 staff also take part in a referral intake/duty rota.
The normal working pattern of this role is Monday to Friday 09.00-17.00, but we have a flexible working policy.

* RMN, SW or OT
Experience
* Working with adults with complex mental health problems
* Working with older adults
* Experience of assessing, care planning and initiating care
* Ability to work under pressure
Knowledge
* Awareness of NHS and national guidelines related to older adult mental health
* Understanding of mental health conditions and dementia
Skills
* Basic IT skills
* Ability to work closely with families and significant others
