Job summary To provide an assessment of care needs of patients presenting with a range of mental health and psychological problems. From this, you will be responsible for the development, implementation and evaluation of programmes of care as part of the MDT. Main duties of the job The post holder will work closely with the Multidisciplinary Team (MDT) within the Trust supporting Later Life patients who present with multiple physical and MH comorbidities.
