Job summary We are looking for an enthusiastic and well-motivated Occupational Therapist to work within the Rotherham Community Falls Therapy service. This is to cover maternity leave for 12 months. This team provides assessment and therapy intervention for adults who have had a recent fall or who are at risk of falling. Intervention is delivered in the persons home environment and if appropriate within a 12 week group settings. You will be delivering a comprehensive falls multi-factorial assessment with clear links to the frailty pathway with access to a medical consultant There are therapy support workers dedicated to the falls therapy team who provide 1:1 and group interventions. There are strong links to community and Functional Fitness maintenance sessions. This post is weekday provision of Monday - Friday, however there are also opportunities to support the wider therapy team over weekends and bank holidays. The successful applicant will have Current HCPC registration, An interest in working in a community based setting, Have an interest in falls and falls prevention, Excellent communication skills, Due to the nature of the role it is essential that applicants are able to travel independently across the county and have access to a suitable vehicle for business purposes. Main duties of the job This team is part of the larger community therapy team and the aim is to provide a high quality, efficient therapy service to maximise patients independence and function within their home environment. Our work supports facilitation of safe and timely discharge and prevention of hospital admission. The community therapy team is a specialist multi-disciplinary team of physiotherapists, occupational therapists and therapy support workers and works closely with a wider MDT of nurses, nursing support workers, social workers, reablement, voluntary sector and care staff.
