You possess a comprehensive understanding of the Care Act 2014 and are skilled at applying social work methods and models to help people achieve independence and manage risks related to abuse and neglect. Your role involves making professional judgments within legal and policy frameworks to carry out assessments, planning, and reviews of care and support needs. You are adept at safeguarding adults, working collaboratively with a wide range of professionals, and supporting service users and carers in planning and executing their care plans. Additionally, you are dedicated to contributing to professional development and service improvement. What the Job Involves The role involves managing a caseload with diverse and complex needs, conducting assessments in line with statutory requirements, and working with individuals to plan and review their support plans. You will actively participate in safeguarding enquiries, collaborate with multi-agency professionals, and ensure integrated and effective working through meetings and joint visits. You will apply the Mental Capacity Act as required and provide tailored support to involve service users and carers in their care. Your responsibilities include monitoring performance, resolving complaints, ensuring compliance with policies, and contributing to the development of services, policies, and practices. Skills Strong knowledge of the Care Act 2014 and relevant statutory instruments Experience with care and support assessments and planning Proficiency in safeguarding adults’ enquiries Understanding of social work theories and application in practice Ability to manage complex and challenging cases effectively Excellent communication and collaborative skills Capability to work independently and collaboratively Competence in maintaining detailed and accurate records Commitment to continuous professional development and learning Registered with Social Work England (or equivalent)
