We are looking for candidates with:
• Relevant NMC registration.
• Previous experience in mental health environments (preferred, but not essential).
• Strong communication, organisational, and timekeeping skills.

• A genuine passion for working with older individuals in the community who are facing dementia and mental health challenges.
Provide tailored support and interventions for individuals with dementia and mental health conditions.
• Collaborate within a multi-disciplinary team to deliver holistic care.
• Monitor and review prescribed medications, ensuring adherence to best practices.

• Collaborate with service users, carers, and the MDT to deliver high-quality care.
• Support medication concordance and ensure safe prescribing practices.
• Contribute to case management, MDT meetings, and CPD.
• Maintain high standards of documentation/record-keeping.
• Travel independently across the county to meet time-sensitive deadlines. A full UK driving licence and access to a vehicle are essential, as public transport will not meet service needs. Please confirm in your application that you meet these criteria.
