Main duties of the job
1. The post-holder will provide clinical expertise, leadership, consultancy and collaborations to ensure that patients gain effective, appropriate and timely care.
2. Will undertake advanced clinical assessment, initiate investigations and interpret their results so clinical diagnosis can be determined and appropriate treatment initiated.
3. Organise the clinical management of individual patients, using highly developed expert clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes in management where required.
4. Conduct advanced health assessments and clinical examination of patients with highly complex presentations and make clinical decisions based on these assessments. Manage patients based upon own interpretation of results
5. Work unsupervised to advise and recommend treatment for patients using clinical judgment, and decision making skills in accordance with agreed local, regional and national policies and guidelines working with your identified mentor
6. In partnership with other clinical teams, collaborate on improving the quality of health-care responding to local and national policies and initiative as appropriate
7. Support and participate in shared learning across the Division and TRFT
8. Contribute to the development of design, implement and evaluate patient involvement and experience and develop effective feedback mechanisms to enhance the service provided by the team
9. The role will involve, but not be limited to you performing the above.

10. The post-holder will provide clinical expertise, leadership, consultancy and collaborations to ensure that patients gain effective, appropriate and timely care.
11. The ACP (post holder) will engage directly with the Consultants, medical staff and the multi-disciplinary team in the management of patients as a pivotal member of the General Surgery Team.
12. The post holder will undertake advanced clinical assessment, initiate investigations and interpret their results so clinical diagnosis can be determined and appropriate treatment initiated.
13. The post holder will use advanced knowledge and skills to provide autonomous practice to patients including independent clinical assessment, diagnosis, and treatment of patients with undiagnosed and undifferentiated conditions.
14. Act as an Advanced Clinical Practitioner demonstrating advanced knowledge and skills
15. To be a pivotal member of the General Surgery Team. Seeing patients in a variety of settings including (but not limited to) new referrals to the specialty, patients seen as part of the specialities clinics, undertaking telephone calls including telephone triage when required, participating in ward rounds.
16. Organise the clinical management of individual patients, using highly developed expert clinical knowledge and advanced skills to assess, diagnose, plan, implement and initiate changes in management where required. This will involve the monitoring and evaluating the response to treatment through analysis and interpretation of clinical tests and investigations and the transfer of care/discharge of patients
17. Conduct advanced health assessments and clinical examination of patients with highly complex presentations and make clinical decisions based on these assessments. Manage patients based upon own interpretation of results
18. Work as an autonomous practitioner, management and prioritising own workload on a daily basis, prioritising patients based on clinical need.
19. Work unsupervised to advise and recommend treatment for patients using clinical judgment, and decision making skills in accordance with agreed local, regional and national policies and guidelines working with your identified mentor
20. In partnership with other clinical teams, collaborate on improving the quality of health-care responding to local and national policies and initiative as appropriate
21. Support and participate in shared learning across the Division and TRFT
22. Assess the impact of policy implementation on care delivery
23. Contribute to the development of design, implement and evaluate patient involvement and experience and develop effective feedback mechanisms to enhance the service provided by the team
24. Recognise and work within own competency and professional code of conduct as regulated by the NMC/HCPC
25. Produce accurate, contemporaneous and complete records of patient consultation
26. Assess effectiveness of care delivery through self and peer review, bench marking and formal audit and evaluation in the quality governance systems and processes of the division.
