What you will do:
1. Operate as an independent specialist practitioner within the multi-disciplinary memory assessment team, ensuring effective assessment, planning, consultation, and diagnosis formulation.
2. Utilise your specialist knowledge of strategies and interventions for treating dementia to deliver exceptional patient care.
3. Confidently identify risk factors and take appropriate actions to address them.
4. Provide a communication bridge between GPs and various services both within and outside the LCFT, striving for the highest standards in patient care.
5. Offer mentorship and clinical supervision to other staff and trainees, fostering a strong, informed team.
6. Actively promote the Trust values within the Memory Assessment Service, adhering to the NMC and HPC code of conduct.
7. Support leadership within the service, contributing to the effective assessment, formulation, planning, and monitoring of care provided to patients and their carers.
