Working alongside Hospital Discharge staff, care homes, individuals, families & carers, you will undertake care coordination, review, and practical interventions within the Renfrewshire area to support discharge planning. This role is based within the Hospital Discharge team, as part of RHSCP Adult Services. Primarily based within Council Headquarters, however also within the RAH where required.
Responsibilities:
1. Work closely with care homes to negotiate and plan hospital discharges in a timely manner, supporting and promoting the partnerships Delayed Discharge agenda.
2. Undertake assessment and reviews for older adults within their own home or within a care environment.
3. Manage demanding and complex services and handle customer enquiries effectively.
4. Develop effective working relationships with managers, staff, and other stakeholders from various backgrounds and disciplines.
5. Demonstrate effective time management, set priorities, and follow processes while being task-focused.
6. Utilize IT packages and manage data through electronic systems.
7. Work as part of a team and on your own initiative, prioritizing your own workload.
Qualifications:
1. SVQ Level 3 in Social Care is essential or be willing to work towards it.
2. HNC in Health & Social Care is desirable.
3. Experience with Eclipse is a plus.
