Old Swan Health Centre Group Practice is looking to recruit an experienced, dynamic primary care pharmacist to join our busy but friendly team serving 9300 patients. We are looking for a practice pharmacist to work closely with the team to ensure we deliver an effective, efficient and quality medicines management service to our patients and our team.
Main duties of the job
As a Prescribing Pharmacist, you will be responsible for efficient prescription signing, medication reviews, and reconciliation, ensuring safe and effective prescribing across the practice. You'll conduct structured medication reviews, working closely with patients and clinicians to optimize treatments, reduce risks, and improve adherence.
A key part of your role will involve running safety searches and implementing medication changes in line with best practice, ensuring compliance with the latest guidelines. You'll collaborate with GPs, nurses, and the wider team to enhance prescribing safety and support long-term condition management.
This role offers a balance of independent clinical work and team collaboration, making it ideal for a pharmacist looking to apply their expertise in a patient-focused setting.

Job responsibilities
Job Summary
As a Prescribing Pharmacist, you will play a key role in medication management, ensuring safe, effective, and evidence-based prescribing for our patients. Your primary responsibilities will include prescription signing, medication reviews, reconciliation, and conducting safety searches to optimize prescribing practices and enhance patient care.
You will work closely with GPs, nurses, and other healthcare professionals to support medication safety, chronic disease management, and prescribing initiatives, ensuring compliance with clinical guidelines and improving patient outcomes.
Key Responsibilities
1. Sign and authorize repeat prescriptions in line with practice policies.
2. Ensure prescriptions comply with best practice, local and national guidelines.
3. Provide clinical oversight on high-risk medications and controlled drugs.
4. Conduct structured medication reviews (SMRs), ensuring medicines are optimized for patient safety and effectiveness.
5. Carry out medication reconciliation for patients following hospital discharge.
Person Specification
Qualifications
1. Registered Pharmacist with the General Pharmaceutical Council (GPhC).
2. Experience in primary care, community pharmacy, or hospital settings.
3. Strong knowledge of prescribing safety, clinical guidelines, and medication optimization.
4. Ability to work independently while collaborating within a multidisciplinary team.
5. Excellent communication and patient consultation skills.
6. Experience in structured medication reviews (SMRs) and long-term condition management.
7. Knowledge of EMIS Web.
8. Experience in conducting prescribing audits and quality improvement projects.
