About the role:
This role supports the delivery of client care under the guidance of qualified Allied Health Professionals. Working closely within a multidisciplinary team, you will assist in providing community-based, low-intensity rehabilitation programs designed to enhance client independence and wellbeing.
In this role, you’ll have the opportunity to develop your skills while working with a diverse range of clients. Key responsibilities include delivering therapy programs focused on improving daily function, enhancing mobility, and providing falls prevention education to reduce risks and promote safety.
Additionally, you will assist with a variety of administrative tasks to support both the clients and the Allied Health team. These tasks may include, but are not limited to:
* Scheduling client appointments
* Coordinating patient transport arrangements
* Facilitating community access days
* Organising in-home fire and smoke alarm checks through local services
* Supporting dental referrals
* Assisting with Mobility Parking Scheme applications
* Participating in client case planning
As part of the role, you will also develop skills in sourcing and setting up prescribed equipment, as well as providing support to clients during outpatient appointments.
You will be well-supported through structured supervision and mentorship, ensuring you feel confident and capable in your work. A comprehensive local orientation program will help you settle into your role, while access to Allied Health Assistant competencies will guide and support your professional development. You will also have the opportunity to complete CHIME training, alongside ongoing learning through My Health Learning and a range of internal and external professional development opportunities.

You will be joining our dedicated multidisciplinary team at Tamworth Community Health Service, where collaboration and support are at the core of all that we do. Working alongside Occupational Therapists, Physiotherapists, Social Workers, Enrolled Nurses, and Registered Nurses, you will deliver low-intensity rehabilitation programs to our clients following an admission to hospital, empowering clients to regain their independence and improve their quality of life.
About You:
We are looking for someone with a genuine passion for providing client-centred care and supporting the wellbeing of the ageing population. You will have excellent interpersonal skills, allowing you to build positive relationships with clients, families, and your team. Organised and proactive, you manage your time effectively and can confidently prioritise client care while balancing administrative responsibilities.
You thrive working both independently and collaboratively, and you are skilled in delivering low-intensity rehabilitation programs, including physiotherapy and exercise programs as prescribed by Allied Health Professionals. Your efficient computer skills support your ability to manage administrative tasks with ease. Additionally, you may hold water safety or hydrotherapy support training, enabling you to assist in the delivery of hydrotherapy sessions.
