The purpose of this role is to actively coordinate and facilitate prompt investigations, diagnostics and clinical review, collaborating with the multidisciplinary team, thereby improving quality of care and experience for patients ensuring timely access to haematology treatment. The post holder will be responsible for the creation and maintenance of a database to ensure that patients who are entered onto the patient stratified follow up pathway are appropriately managed.

Main duties of the job
* Work closely with the multi-disciplinary teams to ensure the efficient management of the patient stratified follow up pathway.
* Assist in the site specific data collection processes, diagnostic cancer pathway waiting times and national cancer data sets and input on various information systems.
* Be responsible and accountable for practice and behaviour under the guidance and supervision of a registered practitioner and by working as part of the oncology multidisciplinary team.
* Provide coordination of high quality patient care through ongoing telephone, face to face assessment and proactive identification for needs using basic knowledge, approved tools and procedures.
* Act as a single point of contact for patients, carers and the healthcare team, triaging enquiries and referring to the appropriate hospital department or community setting.
* Early detection of problems and putting in place action plans to reduce unnecessary in-patient admissions.
* Document and monitor all aspects of patient care coordination of service delivery.
* Support the distribution of any patient experience surveys.
* Triage incoming calls and initiate appropriate responses according to assessment tools, protocols, individual pathways and by liaising with the haematology team.
* Make pre-planned outbound telephone calls to patients to assess their needs and monitor their progress, liaising and updating the oncology team as appropriate.
* Coordinate the necessary assessments, appointments or investigations as identified in the patients care plan.
* Support the delivery of patient information to ensure all patients/carers receive appropriate verbal/written communications in a timely manner.
* Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to care.
* Contribute and coordinate the holistic needs assessment and the development of a care plan for patients with non-complex needs and monitor and review this as required with the patient.
* Evaluate outcomes of care delivery with the registered practitioner.
* Coach patients and carers to understand signs, symptoms or situations to be aware of that would indicate concerns to raise with a healthcare professional.
* Inform patients and carers on how to make contact with the relevant services when they feel their condition or needs have changed, including what to do out of hours.
* Effectively utilise a computer and various software applications in order to enter and maintain patient data, communicate with the multidisciplinary team and complete other IT tasks required by the post.
* Discuss support options with patients and carers sensitively and ensure that the haematology team are kept informed of outcomes.
* Liaise with ward teams prior to a patient's admission regarding any support needs the patient may require.
* Promote patient involvement relating to the provision of cancer care.
* Participate and display a willingness to learn and support cancer patients and develop relevant technical competency.
* Participate in departmental audits as required under the supervision of the healthcare team and input data as requested.
* Be responsible for own learning and knowledge relevant to cancer and maintain compliance with all mandatory training as per Trust policy.
* Participate in regular formal supervision and develop a personal development plan related to the departmental competency frameworks.
* Manage own daily timetable of delegated tasks including time for supervision and training.
* Access and input confidential and sensitive patient information through integrated databases in accordance with the agreed policy and procedure.
* Carry out administrative duties as required by the role.
* Demonstrate an awareness of the limits of own practice and knowledge and when to seek appropriate support and advice.
* Assist the healthcare team to develop education and information resources for the general public and other health professionals regarding all aspects of cancer and cancer prevention.
* Maintain accurate records/documentation of enquiries while respecting patient confidentiality.
Person specification
Qualifications
* GCSE or equivalent level including Maths and English.
* Customer Care qualification or equivalent experience.
Knowledge, Skills and Training
* Experience of working with cancer patients.
* 2 years secretarial experience.
* Knowledge of key access targets.
* Excellent communication skills (both written and verbal) with patients and staff at all levels.
* Able to work effectively as part of a multi-professional team.
* Able to work with minimal supervision.
* Experience of UHNM information systems.
