Wesham Rehabilitation Centre is advertising an exciting post within Mental Health Rehabilitation clinical pathways. This will be a great opportunity for individuals to work as part of this team and initiative.
Wesham Rehabilitation Centre provides a model of rehabilitation care for 24 males, focusing on empowering the service user to gain or recover their confidence and skills that promote independence, autonomy, and encourage hope for the future while in recovery from a mental health illness. We encourage the skills necessary to transition back into a community setting.
We provide a welcoming and domestic feel to service users. The facility is registered with the CQC as an Open Community Rehabilitation service, registered to accommodate service users detained under the MHA. There is access to a full MDT including nursing staff, occupational therapists, psychology, medical staff, and physicians associate.
Applicants must have experience and an expressed interest in working within the rehabilitation pathway.
Main duties of the job
You will be a Nurse working within a multi-disciplinary team and carry out a diverse range of duties. As a Band 5 Nurse within the team, you will be regularly supported and supervised by a nominated Band 6 Nurse and you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
You will actively promote teamwork, respect, integrity, accountability, excellence, and compassion within the clinical team in accordance with the Trust values and Code of Conduct. You are required to maintain links with your professional body and provide evidence for the revalidation process.
You will work within the multi-disciplinary team to ensure the effective assessment, formulation, planning, and monitoring of recovery-focused rehabilitation care directed to service users.

* Documented evidence of continuing professional development (CPD)

* Expert knowledge on mental health issues and how these impact on the individual, carer's, and the wider community
* Expertise in current best practice in Adult Mental Health, and awareness of relevant government directives and legislation
* Specialist knowledge and application of mental health assessments and interventions using expert level clinical reasoning
* Knowledge of current research in the specialist area and ability to implement it into own and others' practice
