The post holder will have a visible presence in the Trust representing the Patient Safety Team and be responsible for leading effective, timely and proportionate responses to patient safety incidents, by working collaboratively and proactively with CMG staff in accordance with Trust, National and Commissioner’s policies and good practice.
The post holder will work autonomously as a Patient Safety Manager with support from the Patient Safety Lead when required.
The post holder will work in line with Trust’s Patient Safety Incident Response Plan and Policy and be responsible for leading proportionate responses to incidents. This will involve working with a range of internal and external stakeholders to review, analyse and use judgement to improve patient safety whilst developing a culture of continuous improvement to deliver high quality care. The post holder will also ensure that Duty of Candour requirements are met.
The Patient Safety Manager will support the Patient Safety Lead to provide Trust-wide support, ensuring high quality reviews and effective learning and improvement as part of the Trust’s annually agreed Patient Safety Incident Response Plan (PSIRP) as required within the Patient Safety Incident Response Framework (PSIRF).

In this corporate role, you’ll be at the forefront of patient safety, collaborating with CMG colleagues, corporate teams, and external organizations. You’ll provide oversight and be a source of support, ensuring that patient safety is always a top priority.
Service Delivery & Development
Your responsibilities will include developing better systems and fostering cultural changes to enhance patient safety. You’ll ensure continuous quality and safety improvements by analysing incident trends and escalating any concerns. When safety incidents occur, you’ll lead and support timely responses using various tools, creating and maintaining action plans to address issues. Conducting reviews and investigations will be a key part of your role, helping to improve systems and processes. You’ll also be responsible for meeting all reporting requirements and timelines, working with experts to manage risks and safety actions, handling complex enquiries, drafting letters, writing reports, and preparing materials for meetings and training sessions.
Governance
You’ll maintain incident management systems, ensuring accurate data for reporting. Supporting evidence preparation for reviews and regularly reviewing incidents to ensure proper grading and resolution will be part of your duties. You’ll ensure compliance with Duty of Candour policies, support critical reviews, share learning outcomes, and help develop improvement action plans based on incident reviews.
Patient/Customer Service
Being highly visible and providing expert advice on patient safety will be crucial. You’ll help healthcare professionals deliver safer care by understanding work systems and culture, identifying factors in incidents, and recommending improvements. You’ll support patients, families, carers, and staff affected by patient safety incidents through ongoing communication and involvement during investigations, following NHS guidelines. Your strong interpersonal skills will help you manage stakeholder expectations and find solutions in emotionally charged situations. You’ll create safe environments that promote a strong patient safety culture, encouraging effective learning and improvement from incident reviews.
Research & Development
You’ll complete required patient safety education and training, assist the Patient Safety teams with implementing the NHS Patient Safety Strategy, and develop safety initiatives. Providing expertise to identify best practices and share learning to reduce patient harm will be part of your role. You’ll conduct thorough, systems-focused patient safety incident investigations using evidence-based frameworks and tools, and support and develop Patient Safety Partners to improve patient safety and review quality.
