Main responsibilities of the role include: To act as an advocate for children and young people with diabetes. Provide an effective, holistic and developmental approach to diabetes care from infancy to adolescence, within the acute and outreach setting and in conjunction with the specialist consultant and the adult diabetes service; providing a cross county service. To support and participate in the assessment, planning, implementation and evaluation of high quality nursing care. Work effectively as a member of a multidisciplinary team. To manage a caseload of patients within a defined area. To act as an autonomous practitioner within the trust, providing a local link to the wider networks for identified speciality. To provide training in all aspects of paediatric diabetes care to all members of multi-disciplinary teams in secondary and primary care.
