Sussex Community Foundation Trust has an exciting opportunity to recruit a Band 6 Physiotherapist to the Hospital Rapid Discharge Team at Princess Royal Hospital, Haywards Heath. This role would suit a practitioner with highly developed leadership and people skills, who can balance clinical and non-clinical tasks within a fast-paced, multi-disciplinary team with a clear vision for the future development of the service and staff. Candidates should be skilled in the assessment and treatment of patients presenting within an A&E department.

Key Responsibilities:
* Comprehend the acuity of patients within the front door setting, understanding and facilitating complex hospital discharges ensuring patient flow.
* Complete patient assessments, considering clinical diagnosis and treatment of patients who may have acute complex and/or chronic presentations, under different specialties.
* Provide an effective high-quality service to all patients and assist with teaching all members of the team including students.
* Contribute to service development within therapies and the wider MDT.
* Implement policy and policy changes in own clinical area and assist with the setting and monitoring of standards of practice.
* Maintain close collaborative working between the multi-disciplinary team.
* Link day-to-day working with UHSx teams within the front door service.

Additional Responsibilities:
* Supervision and Appraisal - Review and reflect on your own practice through regular participation in clinical supervision and annual appraisal with the Team Lead and/or a nominated supervisor.
* Provide clinical supervision for staff at Band 2 to 5.
* Support the operational lead with the appraisal of staff at Band 2 to 5.
* Training staff and students - Support the training and development of other members of the team and students of all disciplines as required.
* Equipment responsibility - Ensure the maintenance of assessment and treatment equipment in satisfactory condition, and ensure careful use and return of equipment, cleaned where appropriate. Report any suspect or faulty equipment to the Team Lead.
* Professional development - Maintain professional registration as a physiotherapist with the HCPC, and show proof of registration to the Team Lead when requested.
* Continual Professional Development - Undertake professional training that may be required by the Trust and engage in relevant activities to meet the objectives identified with your clinical and line managers.
* Personal management - Exercise good personal time management, punctuality, and consistent reliable attendance.
* Quality and Development - Participate in service development, service audit, and outcome measurement to ensure that quality is maintained.
* Best Practice - Keep up to date with current guidelines relating to your field of work to ensure best practice is maintained.
* Undertake clerical duties as required, including inputting information onto IT databases.
