Job Summary:
Salary £25,875 - £28,050 per annum (depending on experience).
Full time 37.5 hrs per week
To work as part of a multidisciplinary Vulnerable Adult Service team based in Norwich at the Inclusion Health hub. The post holder will provide both inreach and outreach support for vulnerable and marginalised adults with severe and multiple disadvantages who may be homeless, living in hostels or in temporary accommodation.
Support will be provided to individuals to register at the primary health care service at the Inclusion Health hub, under one roof and to further access a range of health care services including but not limited to appointments in respect of their health and care needs.
The post holder will, in addition, support vulnerable patients referred from Norwich Inclusion Health Practices as part of the Vulnerable Service model in respect of their health care needs.
The post holder will support the ambitions of Norwich PCN to address health inequalities by providing support services for marginalised groups across Norwich PCN. You will work collaboratively with a wide range of statutory, voluntary and charitable organisations to be able to offer a holistic approach to improve access to health care.
Main duties of the job
The post holder will actively engage on a one-to-one basis with people to provide a comprehensive support package that is sustainable, recovery-focused, and goal-oriented.
The post holder will provide an outreach service working at both the U1R base and a range of outreach venues. They will support and coordinate patients' access to hospital and other appointments in respect of their health and care needs.
The post holder will chaperone where needed during GP, Health and Health Care professional appointments as directed by the clinical team and under the standard operating procedure, following training and enhanced DBS checks.
The post holder will undertake the guidance of the clinical team and under the standard operation procedure for urine drug screening.
The post holder will carry out street outreach as required in collaboration with partners such as the Pathways Norwich service and support people to access the vulnerable adult services at the hub or the Inclusion health practice they are registered at.
The post holder will attend and take an active part in weekly multidisciplinary team meetings and follow up actions and care plans for patients.
The post holder will work closely with the Vulnerable Adult services Integrated care coordinator to plan support needs and coordinate them. This will be in adherence to the guidance within the Vulnerable Adult Service standard operating procedure which requires support for patients to transition to the tiered levels of the service.

Job responsibilities
* Actively seek to safeguard patients within the safeguarding adults guidelines and procedures whilst promoting independence.
* Ensure that risks are identified and managed with the appropriate referrals to risk enablement being made.
* Implement and review support plans as directed by the service for patients and ensuring person-centred delivery of services.
* Work in close partnership with colleagues in the health hub, other agencies, families/carers, to achieve a coordinated service based on the support plan, within eligibility criteria for the Vulnerable Adult Service.
* Ensure that all records/reports, in whatever format, are completed thoroughly and accurately.
* Ensure that own practice is of the required standard by knowing and implementing relevant policies, procedures, and guidance.
* Attend appropriate meetings and forums, relevant to the role, and actively promote the service.
* Attend regularly all staff meetings and other team meetings as directed by the management.

Person Specification
Knowledge
* Good administrative skills, including basic computer/word processing skills.
* A working knowledge of the Welfare Benefits systems.
* An understanding of complex needs of service users.
* Have an understanding of and the ability to implement anti-discriminatory practice.
* Understanding of Safeguarding Vulnerable adults.
* Awareness of good professional boundaries, particularly in relation to working with vulnerable adults.
Skills & Ability
* Ability to fully engage with patients in the management and monitoring of their personal budget, but where appropriate seek assistance, support and direction when confronted with high-risk and complex features of a case.
* Proven ability to communicate effectively, both verbally and in writing at all levels. Good interpersonal skills.
* Ability to work methodically with attention to detail and keep case notes and records.

Experience
* A minimum of 3 years direct experience of working (paid or voluntary) with service users with at least one of the following support needs: substance misuse, offending history, mental health, learning disabilities, single homeless, and a clear understanding of at least two other related groups.
* Experience of lone working with minimal supervision, ability to manage time effectively and possess good organisational skills.
* Experience with self-injurious and aggressive behaviours.
Qualifications
* Relevant qualification in housing, social work, or similar profession. Or relevant experience.
* NVQ Level 2 in Health and Social Care.
Other
* Ability and willingness to be flexible and work some unsociable hours, including evenings, weekends and Bank Holidays if requested.
* Ability to work in close conjunction with Health, voluntary organisations, and other providers partners.
