An exciting opportunity has arisen to work as part of the St Helens Community Cardiac Service. Experience of working within the cardiology environment, with evidence of clinical knowledge and skills of caring for cardiac patients is essential.
The role involves supporting patients following a cardiac diagnosis such as an MI, cardiac bypass surgery or heart failure, to assist in the patient’s recovery, improve their health and wellbeing by providing evidence-based clinical practice and knowledge. The role also includes supporting the therapy team in delivering a cardiac rehabilitation exercise programme.
The successful candidate will work as part of a nurse-led service working alongside the cardiac nurses, heart failure nurses, and therapy staff undertaking a variety of duties. Potential candidates are welcome to contact the service to discuss the role in more detail.
Minimum Requirements:
An Enhanced DBS with barred lists for Adults is required for this post.
Key Duties:
1. To assess, plan, implement, and evaluate care for cardiac patients with a variety of cardiac conditions.
2. To work autonomously and manage own specialist defined patient caseload.
3. To deliver cardiac rehabilitation programmes phase II and III.
4. To be involved in developing specialised, evidence-based service delivery which is linked to both local and national targets.
5. Liaise with all members of the MDT involved in patient care including GP, Cardiologists, etc.
6. Provide clinical supervision and mentorship.
7. Complete a holistic assessment of risk factors and offer appropriate lifestyle advice including exercise and educational components as well as psychological support and lifestyle modifications.
8. Refer patient to appropriate services for ongoing care.
9. Deliver nurse-led clinics and home visits post cardiac event clinics where patients' recovery symptoms and treatments are assessed.
10. Advise/undertake drug titration and post MI clinics.
11. Collection and input of NACR (National Audit of Cardiac Rehabilitation).
12. Work in collaboration in the development and delivery of coronary heart disease training for the Primary Healthcare Team.
Clinical & Professional Responsibilities:
1. Maintain Professional Registration.
2. Adhere to the NMC Code of Practice and The Scope of Professional Practice.
3. Ensure that nursing care is appropriate, provided, and is consistent with clinical guidelines.
4. In accordance with the Health and Social Care Act 2008, the post holder will actively participate in the prevention and control of infection within the capacity of their role.
5. To report and record any untoward incidents.
6. To assist in the promotion of effective communication with all staff, patients, and relatives and other departments as required.
Administrative Responsibilities:
1. Offer clinical supervision to other colleagues.
Teaching & Training Responsibilities:
1. Assist in the development of an education program for key areas.
2. Be involved in staff education and development.
3. Maintain and develop personal knowledge and skills in the field of cardiology nursing.
4. Develop audit skills and undertake training as required.
Research & Audit:
1. Assist with relevant audit projects.
2. Work in collaboration with the audit department to ensure correct data entry.
3. Assist with the development of appropriate audit tools and database.
4. Assist in collecting all the required information.
5. Assist with measures to improve performance as required.
6. To comply with the Data Protection Policy of the Trust.
7. Involvement in research projects as appropriate.
