This post would suit a Nurse with Communityexperience wanting to develop their Practice Nurse Skills, or a Practice Nurselooking to develop Community Skills. Support and training will be given.

Job responsibilities
To work as a Registered Nurse with the Puddletown Surgery NursingTeam to provide evidence-based, high quality, holistic and patient centrednursing care for patients in the practice population, both in the surgerysetting and in their own homes.
KEY CRITERIA
Awarenessof the role of the Primary Care Nurse within the Primary Health Care Team.
Goodcommunication skills and ability to work within a team.
Commitmentto training to acquire the skills needed as a primary care nurse. - including accredited training in chronic diseasemanagement as required e.g. diabetes, asthma,COPD, spirometry.
Abilityto work alone without supervision when trained, following practice protocols,under own initiative, prioritising workload, and effective time management.
Willingnessto be flexible and work nursing sessions according to the practice need tocover for training, holidays and sick leave as required.
MAJOR DUTIES AND RESPONSIBILITIES OF THE POST
PROFESSIONAL RESPONSIBILITIES
Beaccountable for your own professional actions, and work within the boundariesof the NMC Code of Conduct - maintainingcourtesy, discretion, and patience at all times. Maintain patientconfidentiality and respect privacy and preferences.
AttendMDT meetings and training sessions as necessary to ensure professionalcompetency and continuous professional development is maintained.
UndertakeMandatory training as required.
Have anannual appraisal with their line manager, which will include setting personalobjectives and a development plan.
Contributeto the assessment, supervision, development and teaching of junior staff andpre-registration student nurses if required.
Have astatutory duty of care for their own personal safety and that of others who maybe affected by their acts or omissions.
CLINICAL
Assessthe needs of patients in a variety of settings and formulate a plan of care.
This mayinvolve liaison with the wider multi-disciplinary team e.g., GPs, consultants,hospital nurses, carers, social workers, hospice staff and cancer care nurses,voluntary and statutory workers.
Provideevidence-based nursing care in the surgery and community settings.
Evaluateindividualised care packages in all environments, to ensure efficiency.
Proactiveand opportunistic screening for chronic diseases, e.g., diabetes, asthma,coronary heart disease.
Chronicdisease patient management, monitoring, regular reviews and pt education forDiabetes, Asthma, COPD etc.
Recognisepatients readiness to change unhealthy behaviour, supporting health promotion,giving advice and information on lifestyle issues and refer appropriately.
Performvenepuncture.
RecordECG, BP and other observations as required.
Woundcare / Leg ulcer management / Doppler / dressing techniques / removal ofsutures etc.
Earexamination and irrigation.
Performcervical screening procedures and promote breast awareness.
Adviseon continence management and make appropriate referrals.
Catheterisationand bowel procedures.
Providepalliative support for patients and carers at the End of Life, includingadministration of drugs and use of syringe drivers.
UndertakeINR monitoring.
Administeradult and paediatric injections, including immunisations, under anindividualised prescription or Patient Group Direction, ensuring safe storage,rotation and disposal of vaccines and drugs.
Delivera holistic travel health service, including vaccinations and travel advice.
Assistthe GPs with Minor surgery and chaperone during intimate procedures.
Undertakefirst aid and management of emergencies, e.g. burns, haemorrhage,hypo/hyperglycaemic emergencies/asthma attacks.
Undertakeannual training and demonstrate competency in anaphylaxis and resuscitationtechniques
ADMINISTRATIVE
Maintaindetailed, accurate, contemporaneous patient records of care given, adhering toPractice policy and in line with NMC guidelines.
Have aworking knowledge of the practice computerised notes system. (SystmOne)
Contributeto the identification of health needs and service provision for the practicepopulation.
Contributeto the development of policies / protocols / guidelines within the practice.
AssistGPs to help deliver effective management of patient care as detailed by theQuality and Outcomes framework / Frailty and other enhanced services.
SAFEGUARDING and RISK ASSESSMENT
Awarenessof Adult and Child safeguarding - Recognise the signs of child abuse, familyviolence, vulnerable adults, substance abuse, addictive behaviours, suicidaltendencies and be aware of local, referral procedures.
Be awareof risks to your own safety and security whilst working, especially in thecommunity setting.
Carryout your own risk assessments of patients homes for the benefit of the patientand for the benefit of your own safety and report any concerns to your LeadNurse.
INFECTION CONTROL
Supportpatients and colleagues in adopting sound infection control measures.
Applyinfection control measures when collecting and handling of laboratoryspecimens, disposing of waste materials, dealing with blood and body fluidsspillages and report and treat sharps injuries.
