Location: 45-47 Monument, Weybridge KT13 8RN
Liaison: Communicate with clients, clients’ representatives, care workers, Care Managers, and other professionals (e.g., Social Workers).

Requirement: We are looking for a Care Coordinator to work in our office located in Weybridge KT13 8RN.
Main Responsibilities:
1. Ensure care worker visits to clients reflect the continuity and flexibility Hartwig Care promises.
2. Ensure visits occur on time with appropriate care workers tailored to clients' needs.
3. Update care workers with necessary information regarding client visits and company policies.
4. Receive information/reports from care workers within set timelines.
5. Coordinate care worker review bookings and team meetings, ensuring they happen as scheduled.
6. Cross-reference actual bookings against planned bookings for accuracy.
7. Aid in improving service monitoring and client safety.
8. Assist Care Managers in communicating with care workers regarding daily visits and meetings.
9. Answer phone calls and text messages, take messages, and redirect calls.
10. Handle issues regarding client schedules and care worker schedules.
11. Manage complaints, concerns, or compliments through notes and emails.
12. Confirm all rotas have been received and hours accepted.
13. Send the weekly rota to all clients by post.
14. Handle day-to-day allocations of visits.
15. Ensure clients are informed of changes to their weekly rota.
16. Assist in identifying when new care workers are needed.
Ideal candidate qualities include:
1. Valid driver’s licence; business insurance preferred.
2. Interpersonally confident and a proactive team player.
3. Ability to handle confidential matters sensitively.
4. Ability to thrive under pressure with good organisational and multitasking skills.
5. High level of attention to detail and ability to prioritise workload.
6. Excellent communication skills and professionalism.
7. Competence in Microsoft Office products: Excel, Word, Microsoft Outlook.
8. Proactive and able to use initiative during complex situations.
