Clinical Prioritisers - North & East IToCH

The North & East Integrated Transfer of Care Hub (ITOCH) Team is seeking to recruit a Clinical Prioritiser to coordinate local demand and capacity, ensuring that individuals' needs are triaged, prioritised, and addressed through the most appropriate resources from the multi-disciplinary and cross-organisational team. We encourage applications from Band 5 staff looking to develop themselves. This position will be within the Neighbourhood iToCH team based in Oak Tree Surgery.
The Clinical Prioritisers will focus on developing the service through:
* Prioritisation: locality-based decision-making enables effective prioritisation of care delivery across existing team boundaries.
* Capacity: localising delivery and ensuring effective flexibility of workforce improves efficiency of service delivery.
* Communication: enhanced communication speeds up escalation and adds flexibility to response.
Main Duties of the Job
The Clinical Prioritisers will work in a multi-disciplinary manner, utilising RiO to triage and process referrals through the new STRATA system. They will assist in the coordination of demand and capacity to ensure the highest needs are prioritised and addressed with the most appropriate available resources from the multi-disciplinary and cross-organisational team.
The Clinical Prioritiser will provide specialised knowledge and exceptional skills in managing frailty and intermediate care, supporting advanced assessment and care planning for individuals suffering from a range of complex conditions, ensuring a seamless pathway through acute and community services.

Person Specification
Education/Qualifications/Knowledge and Abilities
* NMC registration/AHP registration
* Clinical knowledge and experience in a community setting
* Awareness of national and local health care policies
* Professional nursing or AHP qualification at degree level
* Evidence of relevant postgraduate study or substantial experience in a health care setting
* Teaching or mentoring qualification
Skills and Aptitude
* Computer literate - Microsoft Office applications/email/electronic patient referral and record systems/video conferencing
* Time management
Knowledge and Abilities
* Ability to make clinical and timely risk management decisions in sometimes challenging situations
* Knowledge of various agencies involved in healthcare, including acute, primary, social care, and voluntary sectors
* Clear understanding of clinical/social effectiveness and the need to provide evidence-based care
