The post holder will work independently within the remit of the role but under the guidance and supervision of the Clinical Nurse Specialists to Analytical and Judgement skills Access and input confidential information through integrated databases in accordance with the agreed policy and procedure. Demonstrate a broad understanding of cancer treatments offered within the Cancer Services. Be aware of the particular physical and psychological needs of cancer patients, observe and listen and report any patients concerns, alerting the CNS so that any necessary treatment and or reassurance can be given to maintain patient safety and well-being. This will involve prioritising a patients clinical and psychological symptoms and judging how best to action any concerns when there may not be a straightforward choice of options. Planning and Organisational skills Plan and organise own workload to ensure patient needs are prioritised. Plan and organise own day to day workload, which will also involve liaising with several other healthcare professionals and external agencies. Responsibility for Patient and Client care, treatment and Therapy Monitor and check appointments and diagnostic tests are booked and undertaken in an appropriate and timely manner. Ensure patients receive timely and appropriate information in regard to their diagnosis and treatment and signpost to appropriate supportive leaflets and literature. Respond to telephone queries by a model of triage and appropriate referral to CNS or other allied healthcare professional. Support the delivery of appropriate information to patients in consultation with the multi-disciplinary team and according to any agreed information pathways, including printing of information Responsibility for Policy and Service Development Implementation Participate in and contribute to service development Participate in initiatives to explore patient experience and service feedback Assist the CNSs with the implementation of Treatment Summary Records Participate in the development and delivery of Health and Well Being Sessions for Living with and Beyond Cancer. Support and coordination of remote surveillance for patients on Patient Centred Follow up (PCFU) pathways to ensure risk free survivorship Communicate effectively with relevant MDTs and clinical teams to ensure that all new and migrated PCFU patients are identified for coordinated support as assessed by health professionals. Be responsible for maintaining the IT Tracker Somerset database for patients on PCFU, monitoring clinical alerts, organising relevant tests required and ensuring results are flagged to the named clinician and MDT. Be responsible for the regular monitoring, auditing, and reporting of compliance with the remote surveillance pathways in line with the governance protocols. Contribute to clinical and IT team reviews of the implementation of remote surveillance systems and any issues identified, contributing to solution identification and resolution. Ensure all documentation is recorded accurately, legibly signed and dated in accordance with Trust policies. Act as the patient advocate and facilitator in order to resolve issues that may be perceived as barriers to care. Responsibility for Human Resources, e.g. Supervision, Training, HR Advice and Management Participate in mandatory training and education opportunities for self- development and shared experience. Support colleagues, demonstrating own activities and providing advice as necessary. Demonstrate excellent interpersonal and communication skills. Share knowledge from training days attended and actively participate and contribute to the Practice Development within Cancer Services. Participate and display willingness to learn, develop a relevant technical competency / area of interest and keep up to date with current national and local issues and developments. Responsibility for Information Resources and Administrative Duties Attend MDT meetings, assist with preparation of MDT and provide assistance to the CNS with arranging tests, investigations and outpatient appointments as a result of MDT decisions Use computer skills to document assessments Ensure accurate and timely documentation Coordinate access to patient information and education resources for patients. Facilitate the delivery of Health and Well-Being sessions to patients and carers, as part of the National Cancer Survivorship Initiative. Responsibility for Research and Development To support and contribute to clinical audit and research initiaitives.
