CvSourcers is a leading provider of recruitment and staffing solutions in the Enfield area. We are currently seeking a motivated and compassionate Reablement Officer to join our team and support our clients in achieving their independence and living fulfilling lives.
As a Reablement Officer, you will work closely with individuals who have experienced a decline in their physical or mental health, helping them to regain their independence and confidence. This is a rewarding role that requires empathy, patience, and the ability to build strong relationships with clients and their families.
Key responsibilities:
- Conduct assessments of clients' needs and develop personalized reablement plans to support their recovery and independence
- Provide practical and emotional support to clients, including personal care, medication management, and household tasks
- Encourage clients to participate in activities and exercises to improve their physical and mental wellbeing
- Work closely with healthcare professionals, social workers, and other support services to ensure a holistic approach to care
- Monitor and review clients' progress, making adjustments to their reablement plans as needed
- Maintain accurate and up-to-date records of clients' care and progress
- Communicate effectively with clients, their families, and other stakeholders to ensure their needs are met and their voices are heard
- Adhere to all relevant policies, procedures, and regulations to ensure the highest standards of care and safety for clients
Requirements:
- Previous experience in a care or support role, preferably in a reablement or rehabilitation setting
- NVQ Level 3 in Health and Social Care or equivalent qualification
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Empathy, patience, and a positive attitude towards supporting individuals with diverse needs
- Knowledge of safeguarding procedures and ability to respond appropriately to safeguarding concerns
- Flexibility to work shifts, including evenings and weekends, as needed
- Full driving license and access to a vehicle for work purposes
