At Bradford District Care NHS Foundation Trust (BDCT), we are transforming our community services to ensure we deliver the best care to our patients while supporting staff to do this in a safe, caring, effective, and well-led manner.
We are inviting applications to join our planned community nursing service, which forms part of the Adult physical health community services, delivering care to those people living in their home environment. As a Senior Nurse in the team, you will be working alongside other health professionals, including case load managers (District Nursing), staff nurses, health care support workers, and registered nursing associates.
You do not require a district nursing qualification; however, evidence of post-registration study or a willingness to commit to undertake further study that will benefit the patient population you are working with is essential.
We have opportunities for full-time and part-time posts. In addition, we offer a variety of shift patterns across the service to provide flexibility for staff.
Main duties of the job
The Senior Community Nurse post is predominantly a clinical role with a split of 90% clinical and 10% non-clinical work. As a Senior Community Nurse with a minimum of 12 months experience in community nursing, post holders will utilise their knowledge, skills, and expertise to assess, plan, implement, monitor, and review patient care. Knowledge and skills will be used to develop competencies and clinical knowledge with patients, carers, colleagues, students, and the wider multi-disciplinary team.
Post holders will also support the District Nurse Caseload holder with caseload management and have a key role in communicating important information through the team.

The planned care community nursing service operates between the hours of 8 am and 9 pm, 7 days a week, and a variety of shift patterns exist to cover this time frame.
