Job Summary
We have an exciting opportunity for a full-time onsite/hybrid based Clinical Pharmacist to work with East Kennet PCN due to company growth and expansion. This role is suitable for a highly motivated and focused Clinical Pharmacist who will be responsible for the smooth running of day-to-day activities within general practice, providing clinical support and advice to patients while adhering to safety guidelines.
The successful candidate will have the flexibility to work full-time hours within the core opening hours of the GP practice.
Main Duties
As a clinical pharmacist, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GPs.
You will resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes, and support the improvement of access to healthcare.

Key Responsibilities
As a clinical pharmacist, your key responsibilities will include:
* Confidently acting within professional boundaries, supporting and working alongside a team of pharmacists and GPs
* Resolving medication-related queries
* Polypharmacy medication reviews and SMRs
* Telephone general medication reviews
* Improving patient outcomes
* Ensuring better access to healthcare
* Supporting practice staff with medication queries
* Medicine reconciliation and review post-discharge
* Conducting medical-related audits and working on service improvement with the practice
* Providing expert advice on medications to other clinicians and patients
* Diligently following clinical leadership on medicines optimization and quality improvement
* Ensuring appropriate and efficient use of medication
* Educating patients on medication and disease prevention
* Understanding QOF, CQC, and local incentive scheme work streams
* Creating and following clinical pharmacy programs according to practice policy
Person Specification
To be successful in this role, you will need to possess the following qualifications and experience:
* A minimum of 12 months' experience working within general practice/primary care
* A relevant degree/qualification showing registration with GPHC
* Experience with EMIS or System 1
* Ability to commute to the practice for onsite working
* CPPE Primary Care pathway completion or willingness to undertake it (an exemption route can be sourced for experienced Clinical Pharmacists)
