We are looking to appoint a Social Care Coordinator to work in our Discharge to Assess Hospital Team to work alongside Social Workers and Occupational Therapists. About the role The Discharge to Assess Hospital Team is a multidisciplinary team who work in collaboration with our health colleagues to promote timely discharges from hospital promoting the home first agenda. The Social Care Coordinator role will entail working with our duty team, reviewing and analysing referrals, problem solving and visiting individuals that have been discharged from hospital to ensure their care act eligible needs are being met in a strength-based way. We are looking for candidates who have experience of working with people with care and support needs or have transferable skills and experience.
