Job Title: Clinical Pharmacist Responsible to: GP Lead/Practice Manager Accountable to: Primary Care Network (PCN) Clinical Director Hours: Part-time 25-30 Key Responsibilities Patient-Facing Clinical Work: Conduct face-to-face and telephone medication reviews with patients. Provide patient education on medicine use, adherence, and side effects. Support chronic disease management by optimising medication regimes for conditions such as asthma, diabetes, and hypertension. Address medication-related queries from patients, carers, and other healthcare professionals. Prescribing and Medicines Management: Support repeat prescribing processes, ensuring safe and cost-effective use of medicines. Manage high-risk drug monitoring, ensuring adherence to required blood tests and reviews. Review and action hospital discharge summaries and outpatient letters to ensure appropriate medication changes are implemented. Engaging with Prescribing Quality Scheme (or equivalent) where agreed as appropriate by the practice. Promote antimicrobial stewardship, encouraging responsible prescribing of antibiotics. Identify areas for improvement in prescribing safety through audits and risk assessments. Practice and PCN Development: Contribute to Quality and Outcomes Framework (QOF) and Investment and Impact Fund (IIF) targets. Assist with clinical audits to improve prescribing efficiency and safety. Participate in CQC inspections, ensuring compliance with medicines management regulations. Collaborate with PCN pharmacists and community pharmacy teams to enhance integrated care. Lead and support training initiatives for practice staff on prescribing best practices and medicine safety alerts
