Job summary An excellent opportunity has arisen for an motivated and enthusiastic Discharge Facilitator to support the Thoracic Surgery Day 1 discharge programme at South Tees Foundation Trust. The successful applicant will ideally have previous experience working in a ward setting, specifically supporting AHP teams and is expected to have previous experience of supporting discharge planning. The Physiotherapy Service is a well-established large, progressive and dynamic service, which continues to develop within the Trust. We have an opportunity to explore a comprehensive Day 1 Discharge service where exceptional planning, communication and commitment will be key to its success. Main duties of the job The post holder will be recruited on a 12 month fixed term contract and will be employed as a Day 1 Discharge Facilitator, supporting the Day 1 Discharge Lead for patients who have undergone thoracic surgery. The post holder will provide clinical assessment and support with the rehabilitation and physiotherapy management of the thoracic surgical patients. Alongside the existing thoracic team, they will contribute to clinical developments and research opportunities to proactively move the thoracic physiotherapy service forward to the benefit of patients and staff within the Trust and at a regional network level.
