Job summary
Rayleigh & District PCN is looking to recruit an advanced clinical practitioner to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries.
This role will work as part of this large team that includes advanced nurse practitioners, advanced clinical practitioners, paramedics, care coordinators, social prescribers, pharmacists, pharmacy technicians, nurse practitioners and healthcare assistants.

Rayleigh and District PCN is made up of six GP surgeries covering a population of approximately 56,000 across Rayleigh, Hullbridge and Hockley. Qualifications
Essential
8. Active Health and Care Professionals Council (HCPC) Registration and license to practice.
9. Masters degree in pharmacy (MPharm) (or equivalent).
10. MSc in Advanced Practice or equivalent.
11. Non-Medical Prescribing.
12. Advanced Skills in managing long term conditions.
13. Previous experience working as a senior clinical pharmacist with a minimum of 4 years experience (or equivalent) as a pharmacist, with at least 2 years in general practice.

14. Evidence of recent teaching, mentorship, and supervision of staff.
15. Advanced skills in listening and information processing, alongside empathetic skills to ascertain, understand and respond to individuals complex needs and concerns.
