An exciting opportunity has arisen for a dynamic, forward-thinking band 7 highly specialist Occupational Therapist to join our Place Based Care Team in Newport.
We are making excellent progress in Newport with developing our Place Based Care (PBC) model to ensure patient centred Primary and Community Care is delivered as close to home as possible to support people's health and wellbeing whilst promoting prevention and self-management opportunities.
Main duties of the job
You will work alongside the Clinical Lead Nurses for Place Based Care and collectively provide support to our established team of MDT Coordinators to work proactively with GP Practices and multi-sector partners including via the Virtual Ward /MDT meetings to support and prevent deterioration for patients living with frailty, complex illness and emerging biopsychosocial needs.
Utilising your experience and skills, you will deliver Occupational Therapy to support a range of early proactive multi-professional interventions that avoid health exacerbations, deterioration and presentations to the GP/OOH/UPC/ED for non-medical assistance.

Job responsibilities
Using the model of early identification, preventative interventions and enablement ethos, you will support people by implementing person-centred goals that empower them to access opportunities to optimise their occupational performance, quality of life, and their physical, social and psychological wellbeing which in turn will avoid deteriorations, hospital admission or emergency department presentation.
The successful candidate will need to have the analytical and critical thinking skills to gather and provide ongoing evaluation, evidence and vision for future practice development.
The Post holder will be supported by and will work under the supervision of the Professional Lead Occupational Therapist (OT) for the Borough.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Person Specification
Qualifications & Knowledge
* OT degree or diploma
* Extensive experience in working within the specialist area related to the post.
* Significant experience working at Senior OT level
* Professional knowledge acquired through degree supplemented by further training, short specialist courses, to master's equivalent level (CQFW level 7) or recognised equivalent - at least 3 modules
* Accredited practice placement educator
* Significant experience of supervising less experienced OTs
* Involvement in innovative working practices
* Contributing to development of local clinical guidelines.
* Ability to assess, plan and implement highly specialised treatment and advice
* Ability to travel around geographical areas to meet the requirements of the post.
* Member of relevant RCOT Specialist Section
* Leading the development of local clinical guidelines
* Implementation of national initiatives affecting OT
