We are seeking a Band 5 RN who is keen to work & develop their skills within the community setting. You will work within a Primary Care setting, led by one of our Community Matrons and will work alongside other professionals such as GP's & Social Services. You will be an autonomous practitioner and have excellent communication and interpersonal skills. You will work to a high professional standard, which will enable us as a team to provide high quality, person-centred care.
The Community Nursing service provides nursing care to patients at home across a 24 hour period with a Day Team (0830-1630) and OOH's team (Twilight 1400-2200, Nights 2130-0830) seven days a week 365 days a year.
Successful applicants will have good communication and interpersonal skills and will need to be flexible and adaptable. Community experience is desirable although not essential as we have an innovative competency-based approach to training and development to support you to maximise your career potential. In return for your skills and commitment we will offer you regular supervision, internal and external training opportunities, and regular contact with other professionals across the area.
You must be a car driver with a valid driving licence or have access to transport with appropriate business insurance in order to travel throughout the Trust, to meet the needs of the service.
Main duties of the job
The post holder will take responsibility for managing, co-ordinating patient care and utilising other services.
You will participate in inter-professional working and the development, implementation and evaluation of the delivery of health care services.
Safely manage high risk areas including:
1. Lone working in the community
2. Patient handling in challenging environments where the principles of best practice regarding patient handling may be compromised.
3. Driving self
4. Handling medical sharps and adhering to infection control policy
5. Handling body fluids and other hazardous materials
You will have responsibility for providing the highest standards of patient care through ongoing assessment, planning, implementation and evaluation of programmes of care.
Working with the District Nursing team and other agencies you will use your advanced person-centred assessment skills for the health and well-being of patients with complex needs.

Additional Criteria

* Is able to work legally in the UK.

* Ability to travel daily for meetings across a range of sites in Southampton/Portsmouth/Hampshire.
* For posts which require a professional registration, postholders hold a valid up-to-date registration with their professional body.
* For posts where postholders are required to drive as part of their role they hold a valid full driving licence which enables them to drive in the UK.
