Job summary Wigan Later Life and Memory Service are looking to recruit a Band 6 Community Mental Health Nurse to join a busy Multi Disciplinary Team. The post offers the opportunity to work within the CMHT or the Memory Assessment/Treatment Service (dependant on service needs). Training/shadowing opportunities will be provided. There is the opportunity for professional development within this role. Regular Management and Clinical Supervision is provided. Main duties of the job An opportunity has arisen for a qualified and experienced individual to join the busy and vibrant later Life and Memory Service pathway in Wigan. The pathway comprises of the memory assessment and treatment service, the care home liaison team and our older adults CMHT.The successful candidates will be required to fulfil the role of a CPN as part of a multi-disciplinary team; the care co-coordinator role is an essential component of the role. The team operates from Claire House Health Centre in Wigan. You will be responsible for the management of a case load of clients with complex functional and organic mental health issues and associated social care issues, for which risk assessment and risk management is an essential requirement. In addition you will also assess people with memory problems and offer support following diagnosis.
